Home button doesn't work when pressed and the headphone jack?

I have had my iPod touch for going on 3 years, and my home button won't work when pressed. I have to turn up iPod on and off to get anywhere. Also, my headphone jack works, but it is really hit or miss to get it to work. Thanks for any help you can offer.
