In most cases it´s only the thin metal thing that is lost. First check if it´s possible to adjust the volume by pressing a match or small screw driver. If that works the volume button is ok. Buy a new one and then separate it from the plastic part and glue it back on the iPhone. You don´t need to take the iPhone apart.
This worked fine for me.

what glue did you use?
Thanks for the idea fixing the volume button with glue. Easy, simple, best!
As it is very difficult to release the whole volume button, I guess it's impossible to loose the whole button too. Only the "metal" part gets lost! So don't waste your time to replace the plastic thing because it still sticks in.
I bought a replacement button from ebay and removed the chrome piece from the one I purchased, Applied a dab of loctite and voila fixed :)

I did the same, My volume came off, so I glued it back. So, now it's back on, true. But it's useless as the whole button is glued and I can't move the volume up or down at all (everything is glued and motionless). Can anyone help me please??? :-(

rembito, you may just have to replace the volume button. Follow the links given by "Crisb"
Wait, where did you guys glue it? Across the entire bottom of the piece? It's a moving button, so I'm worried glue will make it unusable...

Mine fell off for the second time now, the first time two months ago I glued it I put too much SuperGlue and one of the sides was hard to press, but it still worked.
Now after all this time it has fallen again, and this time I glued it with a very small drop of superglue right in the middle so that only the "axis" is glued. Making sure the glue only touched the metal part and the rubber, never the "walls" (the case of the phone).
Its better to put very little glue and have it falling once every two months than trying to glue it completely and ruining it like it happened to poor rembito up there.
