Disk Read Error, Please Help

Okay, so I have a PlayStation 2 (slim model). It turns on and stuff like that. My problem is while I am playing my game, the game will freeze and the screen will say “Error To Read PlayStation 2 DVD”. I think that I need a new eye but I am not sure. Does anybody else know what this is doing this or have the same problem I am having?
