Why does my PlayStation keep going into safe mode?

I’ve factory reset the play station twice and the second time I went to the setting and tried to up date went in and checked that the system is fully up to date to the 7.00 but every time the game shuts off when I turn it on it’s telling me to update it to the 7.00 in safe mode. So I click update and it tells me it can’t start ps4. So idk if it’s just me or like am I doing something wrong? I’ve checked the cables cleaned it out made sure everything else is working properly.
