Usually it’s either bad mounting of the clamp (could be not screwed on tight and/or upside down) or no thermal paste / thermal paste wasn’t replaced after disassembling the PS4.

Also need to ensure all thermal pads for the VRAM chips are present near the APU.

Make sure to have thermal paste ready and clean up the PS4 up to the fan as this is strongly recommended:

I have replaced the thermal paste..but its from a nearby computer shop..will that be a reason that this problem is caused?

Will there be any empty screw places..because after reassembling the ps4 i dint see screws that came extra

von

Consoles need good thermal paste if it's some no-name brand one or cheap quality it's not going to provide good enough heat transfer. Arctic Silver 5 is what I recommend for consoles.

Thank you...but not having some screws on place can occur this problem yea..?

Yeah there are also some torx screws not just the two phillips for the clamps that hold the backplate that applies pressure on the vram thermal pads. If those are missing it won't provide good thermal dissipation for the thermal pads.
