You'll need to replace the display assembly, which consists of the LCD, touch screen and front glass. While it is possible to replace just the front glass, to do so properly requires a vacuum chamber, a UV chamber and a vacuum table, so it's not practical for us home users to do for ourselves.

The screen replacement procedure is fairly standard on your phone; remove the back cover, unscrew the midframe cover and remove the battery. Then you separate the old display from the frame, clean off the old adhesive and glue on a new screen. This video shows you step by step how to do that.
