I had the same problem on my ZTE axon and figured the headphone jack was broken. I installed the app "Lesser AudioSwitch" from the play store, which allowed me to force the audio to the headphones. Even though the icon still doesn't pop up, I can play audio through my headphones again. Maybe give that a try?
