It looks like the cap is starting to unthread. Follow Die drehbare Kappe am 4 mm iFixit Präzisions-Bithalter wieder anbringen. to get it back on. Personally I would take it off completely and put a few drips of Loctite red on the threads to hopefully keep it from unthreading again.
