Why is my acceleration so jerky?

When I hit the gas pedal it goes from zero to ten really fast and it feels very jerky.  There is also usually a second to two second delay.  Is this normal? How can I make it run smoother?

General info: over 200,000 miles on it, regular oil changes (full synthetic), well maintained. Honda Civic 2006
