Same thing happened to my iPhone 11 with a Ram mount. Had a 3 days trip with my Yamaha Tracer. I thought I might have dropped it and caused some mechanical in-lens issues, but apparently not … This is $@$*. How can you not mount your phone on a motorcycle?! Obviously the camera app wasn’t even started/recording. Used it for waze & music.
